With Microsoft Forms, you can build survey forms and easily share them with students, parents, and colleagues. To get started with building a survey, follow these steps:
1. In your web browser, navigate to the Microsoft Forms site at http://forms.microsoft.com.
2. Sign in to your Office 365 Education account (school email address and password).
3. UnderMy Forms, clickNew to begin creating your form.
4. Enter a name for your form. You can also enter an optional subtitle for your survey.
NOTE: Form names can contain up to 90 characters and a subtitle can contain up to 1,000 characters.
5. ClickAdd Question to add a new question to the form. You can choose to addChoice,Quiz,Text,Rating, orDate questions.
You should only useQuiz questions when you want to build a quiz form.
NOTE: Your form is saved automatically while you create it.
6. For choice questions, enter the text you want to display for the question and each of the choices.
Want to add more choices? ClickAdd Optionto add more choices than the default two choices. ClickAdd "Other" Optionto add a choice option that displays the text Other. To remove a choice, click the trash can button next to it. You can also choose to make a question required or allow multiple choices for a question by changing the settings at the bottom of the question.
TIP: Click the ellipses button (...) and then selectSubtitleto add a subtitle for a question.
7. ClickAdd Questionto add more questions to your form. To change the order of the questions, click theUporDown arrows on the right side of each question. For text questions, select theLong Answeroption if you want a larger text box displayed on the form.
TIP: To copy a question, select it and then click the Copy Question button in the upper right corner:
8. Text questions also allow you to set restrictions when you need to have numbers as the answer. Click the ellipses button (...) and then clickRestrictions. You can choose to restrict the numbers by selecting from the various options such asGreater than,Less than,Between, and many others.
9. ClickPreview at the top of the design window to see how your form will look on a computer. If you want to test your survey, enter answers for the questions in preview mode and then clickSubmit.
10. ClickMobile from the top right of the screen to see how your form will look on a mobile device.
ClickBack when you are finished previewing your form.
